This study is focused on improving kidney health for people with chronic kidney disease (CKD). **Chronic Kidney Disease** is when the kidneys are damaged and can't filter blood well. The study will test a new method called **Kidney Health Coaching (KHC)** to help delay the need for treatments like **Kidney Replacement Therapy (KRT)** or the use of a **central venous catheter**, which are medical treatments for severe kidney failure.

Participants will receive coaching from trained health coaches who have experience with CKD. The coaches will help patients manage their health and make decisions about their treatment. The study will last for 6 months, and patients must be African American or Black, have certain kidney function results, and have been seen at Emory University Hospital-Midtown recently.

  • Eligibility includes specific health criteria and excludes ongoing dialysis or transplant evaluations.
