**RESTORE Study** tests two ways to help people with weak bones after a fracture. **Augmented-FLS** involves a patient helper, called a navigator, who guides and connects patients to bone health doctors. **Enhanced Usual Care** gives patients and their regular doctors, known as Primary Care Providers (PCPs), information and encouragement. The study also looks at how age, race, gender, income, location, and the time after a fracture affect these two methods.
