**Comprehensive Assistance and Resources for Effective Diabetic Foot Navigation Study**
This study looks at a special program called CARE-D-Foot-Nav. It's designed to help people with diabetic foot ulcers (DFUs). DFUs are painful sores on the feet that can happen to people with diabetes. If not treated well, they can lead to serious problems like amputations. This study wants to see if this program can help these sores heal better in 20 weeks compared to regular care.
In this study, 270 patients with DFUs will be split into two groups. One group will get usual care, and the other group will join the CARE-D-Foot-Nav program. Those in the program will have a helper called a patient navigator. This person will guide them, offering diabetes education, help with transport, and connect them to important health resources.
