This study is about treating proliferative diabetic retinopathy (PDR), an eye condition caused by diabetes that can affect vision. Participants will be randomly assigned to one of two treatments: **faricimab + PRP** or **vitrectomy + endolaser**. **Faricimab** is a medicine that helps reduce blood vessel growth to protect vision. **PRP** stands for pan-retinal photocoagulation, a laser treatment for the eye. **Vitrectomy** is a surgery to remove gel from inside the eye, and **endolaser** is a laser treatment done during surgery.

This study will last for 3 years, and researchers will check vision and other factors like treatment side effects and costs. To join, you must be 18 years or older, have diabetes, and show signs of PDR needing treatment. People with certain health issues like kidney disease, high blood pressure, or who are pregnant cannot participate.
