Heart Failure is a condition where the heart doesn't pump blood as well as it should, leading to symptoms like shortness of breath and fatigue. This study uses a special telephone system called Symptom Care at Home - Heart Failure (SCH-HF). The system lets patients report their symptoms and get advice on managing them at home. This helps patients recognize symptoms early and avoid unnecessary hospital visits.
The SCH-HF system is being tested to see if it's helpful for heart failure patients. The study divides participants into two groups: one receives regular care with daily checks, and the other gets the same plus coaching through the SCH-HF system. To join, participants must have access to a phone and understand English. They shouldn't be on hospice care or waiting for a heart transplant.
