**Diabetic Foot Ulcer (DFU) Study Overview** This study is testing a program called **CARE-D-Foot-Nav**. It aims to help people with diabetes who have foot ulcers. Diabetic foot ulcers are sores that can be serious and sometimes lead to amputations. To help, the study provides a **patient navigator**, a helper who will guide and support patients through their treatment. This person might be a diabetes educator, registered dietitian, or medical assistant. They'll have 30-60 minute meetings with participants once a week for 20 weeks, either by phone or in person. They will also offer extra help like transportation or wound care supplies if needed.
