Tailored Electronic Intervention to Improve Therapy in a Diverse Cohort of Patients With Heart Failure

Heart Failure Study: This study is testing a new way to help doctors use the best medicines for heart failure with reduced ejection fraction (HFrEF). HFrEF is a type of heart problem where the heart doesn't pump blood as well as it should. These medicines are called guideline-directed medical therapy (GDMT), which include special types of drugs like SGLT2 inhibitors and ARNI. The study will see if using a tool called the EPIC-HF checklist in heart clinics helps doctors give these medicines more often. The checklist helps patients compare their medicines to the best doses recommended. This study lasts for one year, split into two 6-month parts. Patients will get the checklist before appointments in the first 6 months. In the next 6 months, doctors will also get extra help to adjust the medicines.

  • Eligibility: Must have a heart failure diagnosis and certain heart test results.
