This study is for Black or African American men who have had treatment for prostate cancer, which is a disease where cells in the prostate grow uncontrollably. The study aims to compare two types of survivorship care plans (SCPs), which are guides that help patients manage their health after cancer treatment. The first type, called a standard SCP, follows a guide by the American Society of Clinical Oncology (ASCO). The second type, a tailored SCP, is designed to be easier to understand, especially for those with low literacy, meaning it uses simpler language and includes educational materials. The study will see if the tailored SCP helps patients understand their care better and access services more easily. Participants will be split into two groups: one receiving the standard SCP and the other receiving the tailored SCP. Each study session lasts between 30-60 minutes.

  • Participants must be Black or African American men who are disease-free after treatment.
  • Excludes those with dementia or who can't understand English well.
  • Participation involves a single session lasting 30-60 minutes.
