Tailored Survivorship Care Plan to Improve Understanding and Access to Survivorship Care in Black or African American Patients With Localized or Oligometastatic Prostate Cancer

This study is for Black or African American men with prostate cancer that hasn't spread far. It tests if a special care plan (SCP), designed for easier understanding, helps better than a regular one. An SCP is a summary that includes past treatments and future health advice. The usual SCP follows top cancer society guidelines. The special SCP adds simple educational materials for those with low reading skills, aiming to improve understanding about care and health decisions. Patients are split into two groups: Group I gets both the regular SCP and the special one, while Group II only gets the regular SCP. The plan is to see which helps more in understanding care and managing health. Participants must have completed main treatments 1-4 years ago and not have other serious health issues. Interviews last 30-60 minutes.
