This study aims to improve diabetes care at the Midtown General Internal Medicine Clinic. Diabetes is a condition where the body has trouble controlling blood sugar levels. The clinic will have special days with trained staff to focus on diabetes care. They will also help with other life issues that can affect diabetes, like not having enough food or a safe place to live, learning about healthy food, exercising, and mental health. The study will train medical residents, or doctors in training, to help with this care too.

The clinic is trying a new way to help patients because many Black and Hispanic patients have more diabetes problems than others. This new way includes doctors, dietitians, educators, therapists, and social workers all working together. They will use new tools like continuous glucose monitoring (CGM), which checks blood sugar levels all the time. This helps patients who can't easily visit special diabetes doctors.

  • The study is for adults 18 and older at the clinic with a high blood sugar level (HbA1c ≥ 9%).
  • It's not for pregnant women or those already seeing a diabetes specialist.
  • Residents at Midtown Primary Care will learn through this model.
