The Health Advocate for Liver Transplant (HEAL-Tx) Transition of Care Pilot is a study aimed at helping young people who had a liver transplant move their medical care from children's doctors to adult doctors. A Health Advocate is someone who helps organize medical care, offers financial help like taxi vouchers, and connects patients to community resources. This can help patients take better care of themselves and improve communication between families and healthcare providers. In this study, researchers will see if having a Health Advocate is helpful for young liver transplant patients.

  • The study is for patients aged 18-25 who had a liver transplant more than a year ago.
  • Participants need a working phone and a smart device for virtual visits.
  • Patients must be able to read and write in English or Spanish.

Note: The study won't include patients with serious thinking problems, those who don't meet age criteria, or those who are non-English/Spanish speakers.
