Phase 2

This study focuses on helping Veterans with heart problems get better at home using virtual tools. Cardiac Rehabilitation (CR) means doing exercises and learning about heart health after heart issues. Instead of going to a hospital, Home-Based Cardiac Rehabilitation (HBCR) lets you do this from home. The study adds digital coaching to the existing system, offering extra help through social media, like group chats and video calls. This helps Veterans stay active and healthy long-term. The study will compare two groups: one doing HBCR as usual and the other with added digital coaching. The goal is to see if digital support helps Veterans finish more sessions and improve their health. Veterans will be randomly chosen to join and will participate for 3 months. They will use a special app for social support, education, and motivation.

  • Study lasts 3 months with digital coaching.
  • Participants will use a mobile app for support.
  • Must speak English and have no serious health risks to join.
